If plans are approved, the boozer could welcome a new games room and improved beer garden.
The proposals have been lodged with Sunderland City Council for The Guide Post on Ryhope Street South and aim to enhance the pub’s offering with a dedicated games area and other improvements.
Plans include a ground floor extension connecting the main building to an outbuilding, which would be converted into a games room with seating.
The outbuilding’s frontage would also be updated to match the pub, alongside new window openings and upgrades to the beer garden.
A planning, design and access statement submitted with the application highlights wider industry challenges, including changing consumer habits and rising energy costs.
It said: "Pub businesses are having to improve their offer in order to remain economically sustainable.
"The works at The Guide Post are intended to do precisely that."
If approved, the scheme could also create six new part-time jobs.
The proposals, submitted by pub group Dorbiere, also include bringing a "currently vacant, dilapidated building that adjoins the site" back into use, which the applicants say would improve the street scene.
Upgrades to the beer garden would include timber lean-tos for covered seating.

The planning statement said: "It is clear that the proposals put forward are sustainable by improving an existing local facility on land that is previously developed."
"The proposals represent an extension to an existing public house which lies within the local centre in the heart of the town.
"The proposals will enhance the local centre due to a significant investment into an existing business."
A decision on the plan is expected following a consultation.
